Care Manager RN - Panorama at Providence Health and Services summary: The Care Manager RN is responsible for managing and coordinating care for patients with chronic illnesses across multiple providers to improve health outcomes and quality of life. This role involves patient assessment, education, and resource coordination within an ambulatory care setting, operating under nursing standards and guidelines. The position requires a registered nurse license, experience in complex patient management, and collaboration with healthcare teams to support evidence-based, cost-effective care. Description The Care Manager RN is responsible for managing patient populations with chronic illness and/or multiple comorbidities. This individual works in coordination with multiple PCPs and serves as the central link to an identified population of patients. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the RN Care Manager Navigator assists patients to gain self-efficacy/management skills, achieve optimum functional health status, and quality of life. Assists patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes. This person works under minimal supervision within the general policies and standard practices and the nursing code of ethics, including Chapter 18.88 RCW. Required Qualifications: Graduation from an accredited school of nursing Upon hire: Washington Registered Nurse License 2 years experience in RN role Preferred Qualifications: Bachelor’s Degree in Nursing Experience in managing complex patients in an ambulatory setting Medical Home Model experience Why Join Providence? The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
