Founded in 1971, we believe that the quality of your health care should not depend on how much money you make, what language you speak or what your health is, because everyone deserves great care.Position Summary:Responsible for providing clinic-based nursing services providing direct age-specific and culturally appropriate care through the use of the nursing process, based on evidenced base care, serves as a clinical resource as a per diem nurse.As a professional Nurse Clinician you will perform both independent nursing functions & delegated medical functions in accordance with accepted practice standardsYour contribution to the team includes:* Follows the registered nurse standards of practice to properly assess each patient to ensure appropriate medical care is delivered in the most optimal, cost effective, and culturally sensitive manner* Identify and assess high-risk patients using established criteria, coordinate multi-disciplinary teams and resources to support patient care needs, provide care coordination/management for chronic medically complex or fragile patients across the life span* Actively participates in the team care of patients with complex chronic diseases, coordinates care of complex patients, provides preventative health.* Working closely with primary care teams, specialty care teams and medical providers, the per diem Nurse will establish a collaborative plan of care to assure adherence to the medical plan, improvement in functional status, and improved ability to self-manage.* Collaborates with other health care team members regarding plan of care, utilization of clinical protocols variances in plan implementation, and expected goal/outcomes to optimize health which can include but not limited to:* medication reconciliation,* insulin management,* Hypertension assessment,* INR management, and* initial obstetrical pre-natal intake* Assess, provide recommendations and next steps for walk-in and telephone call triage patients evaluating acuteness of need and/or priority of intervention and document assessment information of all activities.* Provide assessment, treatment and follow up related to basic wound care for patients with a range of complex medical conditions.* Inform and follow up with patients related to abnormal lab test and x-ray results as needed.This is not an exhaustive list of all duties and responsibilities.Must have's you'll need to be successful:* Associate Degree in Nursing with a minimum of five (5) years' experience.* Current Unencumbered Washington State Registered Nurse license required.* Current BLS certificationIf you know about the following it's a plus:* Bachelor's degree in Nursing from a four-year college or university highly recommended and three to five years related experience preferred.* Experience in a community health center experience is ideal.* Certification in case management highly desiredThe Rich Benefits and Perks:* Paid Sick Leave* Retirement plans with company match* Growth and development opportunities* Recognition Program* And much more!To learn more about HealthPoint, go to www.healthpointchc.org!It is the policy of HealthPoint to afford equal opportunity for employment to all individuals regardless of race, color, religion, sex (including pregnancy), age, national origin, marital status, military status, sexual orientation, because of sensory, physical, or metal disability, genetic information, gender identity or any other factor protected by local, state or federal law, and to prohibit harassment or retaliation based on any of these factors.
