Job Description:

TITLE: Community Health Facilitator

JOB OVERVIEW: Enhance the patient experience through facilitation/coordination of care, informing and educating patient on community resources and striving to improve continuously enhance and improve the "global" patient experience.

ROLE: Refer to Administrative Partner job description.

AREA OF ASSIGNMENT: Clinic Network

RESPONSIBLE TO: Clinic Manager

PREREQUISITES:

* High school graduate or General Education Diploma (GED) preferred.

* Minimum 2 years experience in an ambulatory care setting.

* Good to excellent computer skills including the ability to successfully navigate between multiple applications.

* Preferred Medical Terminology

* Personable, excellent communication skills.

* Care Coordination or Patient Navigator training completed within 6 months of employment.

QUALIFICATIONS:

1. Demonstrated ability to function effectively and interact positively with patients, peers and providers at all times.

2. Demonstrated ability to access, analyze and apply concepts associated with protocol, policy and guidelines.

3. Demonstrated understanding of agency compliance standards.

4. Demonstrated ability to understand and work with a specific set of care parameters

5. Neat and well-groomed appearance, business professional.

6. Demonstrated ability to maintain a calm demeanor at all times.

7. Demonstrated ability to function effectively in an environment where it is necessary to perform several tasks simultaneously, and where interruptions are frequent.

8. Ability to identify, access and assist the patient in connecting with community resources.

9. Demonstrated ability to coordinate care for patients and their families.

10. Demonstrated ability to speak, spell and utilize appropriate grammar and sentence structure.

11. Demonstrated ability to work collaboratively as a team member.

12. Sensitivity to a variety of ethnic, cultural, social, and economic backgrounds and with varied medical and developmental needs.

13. Ability to work independently and meet the goals and objective of the position.

14. Ability to independently organize, set priorities; produce accurate work, and meet deadlines.

PERFORMANCE RESPONSIBILITIES:

* Respond and promote concepts of the medical home.

* Learn, participate and understand concepts of the Medical Home. * Enhance the patient care experience

* Engage the patient in care

* Empower the patient with knowledge and access to support resources

* Improve health outcomes for patients

* Improve the patient and family experience of care

* Improve primary care team satisfaction

* Participate in team change package, measures and associated collaborative goals and objectives

* Provide information and access to supportive counseling as appropriate including the following: * Crisis intervention

* Referrals to internal and external resources

* Contact relevant community agencies to increase knowledge of services available for patients and families.

* Educate per protocol

* When indicated, provide assistance with/advocate for patients/families.

* Provide accurate information to patients, physicians, staff and community as requested in a timely fashion

* Take responsibility for professional development to maintain and increase competency and support the education of patients, families and staff: * Participate in ongoing education relevant to role

* Provide guidance to staff for dealing with difficult patient and family situation

* Establish contact with community resources and develop tools to educate peers on patient advocacy and support networks

* Maintain confidentiality of patient information and medical records.

* Demonstrate awareness of the importance of cost containment for the department.

* Perform other duties as assigned.
